Once you make an appointment, we will send you an email from our scheduling software, Jane, confirming your appointment. You can fill out the new patient forms on a computer prior to your appointment or in our office if that’s easier. Just arrive about 15 minutes early and we can give them to you.
If you need to change or cancel your appointment, we ask for 24 hours’ notice as a courtesy to avoid the $45 late cancellation fee.
We’re located at 1630 W. Division, near the corner of Division and Ashland. Please note that ONLY street parking is available- don’t park in the lot behind our building (it’s Spot Hero parking). We are also conveniently located just down the street from the Division Blue Line stop if taking the CTA.
